Camp Waiver
It is the responsibility of the campers’ legal guardians to ensure that the camper is healthy and
has no physical problems which would prevent the campers’ participation in camp activities.
Responsibility for primary medical insurance coverage rests with the camper.
I certify that I am the parent or legal guardian of the camper. I authorize any medical treatments
that might be advised by physicians or athletic trainers available to my son/daughter/ward
while present at camp.
I understand that participation in physical activities will occur during camp, and that injuries can
take place during such camp activities. I understand that, as with any sport, injuries can occur,
and we hereby acknowledge that our child is physically fit and mentally capable of participating
in camp activities. I represent that I have sought the opinion of our child’s pediatrician and
he/she concurs that my child is fully capable of engaging in this sport’s activity, and I am
confident that he/she is able to engage in such a sport.
I am voluntarily choosing to engage my child in this Camp. Should my child contract COVID-19
or have symptoms consistent with a COVID-19 diagnosis, I (We) understand and agree that I
(We) will not participate in the Camp unless directed by a medical provider that I (We) may
participate.
I understand that this camp is sponsored by the party named during registration. I further
understand that Presbyterian College is not a sponsor of the camp, nor does it supervise the
camp in any way.

Photo Release
I, the undersigned, do hereby grant to Tiffany Sardin Basketball Camps, LLC , its employees,
officers, agents, representatives, trustees and assigns my permission to record my
son’s/daughter’s/ward’s photographic image and/or that of my child(ren) (by film and/or video),
and comments (by tape and/or transcription), together with the right to use, publish, copyright
and reproduce in whole or in part any such photographic images and comments as described
above for use in promotional materials, whether the use of above materials be for public
relations, recruitment, development, or any other legitimate purpose of Tiffany Sardin
Basketball Camps, LLC. I hereby waive any right that I may have to inspect or approve any such
photographic images and comments or completed products, which incorporate all, or part of
any such photographic images and comments.
I hereby voluntarily release and hold harmless Tiffany Sardin Basketball Camps, LLC from any
and all liability arising out of or in any way related to the use of such photographic images and
comments, including but not limited to any liability arising by virtue of any blurring, distortion,
alteration, illusion, editing, or use in composite form, whether intentional or otherwise, that
may occur in the making or processing of the finished product.
