No Passport Needed: U.S. citizens can travel to Puerto Rico without a passport.

Liability Waiver and Release Agreement
Hardball HQ Baseball Camp

This Liability Waiver and Release Agreement (“Agreement”) is entered into by the undersigned participant and/or their parent or legal guardian (if under 18 years of age) in favor of Hardball HQ LLC, the City of Aguadilla, Cool Breeze Academy, Hazer Baseball, and their respective owners, officers, employees, coaches, contractors, representatives, agents, and volunteers (collectively, the “Released Parties”).

Acknowledgment of Risks
I understand that participation in the Hardball HQ Baseball Camp involves inherent risks and hazards, including but not limited to the risk of injury, illness (including communicable diseases such as COVID-19), property damage, or other harm that may occur while participating in activities such as throwing, hitting, running, and other baseball-related activities. I acknowledge that these risks may arise from the actions or negligence of participants, coaches, volunteers, equipment, or conditions at the camp facilities.

Assumption of Risks
By signing below, I voluntarily assume all risks associated with my or my child’s participation in the Hardball HQ Baseball Camp, including but not limited to those described above.

Release of Liability
In consideration of my or my child’s participation in the camp, I hereby agree to fully release, discharge, and hold harmless the Released Parties from any and all claims, liabilities, demands, damages, or causes of action arising out of or in connection with participation in the camp, including any claims arising from the negligence of the Released Parties.

Medical Authorization
In the event of a medical emergency, I authorize the camp staff or their designees to arrange for medical care deemed necessary for me or my child. I understand that I am responsible for all costs associated with such medical care.

Photographic Release
I grant the Released Parties permission to use photographs, videos, or other media of me or my child taken during camp activities for promotional purposes without compensation or further consent.

Compliance with Rules
I agree to abide by all rules and instructions provided by the camp staff and understand that failure to do so may result in dismissal from the camp without refund.

Acknowledgment of Governing Law
This Agreement shall be governed by the laws of the state of [Insert State], and any disputes arising under this Agreement shall be resolved in a court of competent jurisdiction in [Insert County/State].

Binding Agreement
I acknowledge that I have read this Agreement, fully understand its terms, and agree that it is binding on me, my heirs, assigns, and personal representatives.
