Dave Gage & Stevenson University President’s Day Baseball Clinic – High School

WAIVER, RELEASE OF LIABILITY AND CONSENT
As the parent/LEGAL guardian of                                                     (Attendee), I hereby grant permission for Attendee to participate in the above-named Baseball Camp/Clinic at Stevenson University and represent that attendee is physically able to participate for the duration of the activity. In consideration of the Attendee  being allowed to participate in the camp/clinic, I, on my own behalf, on behalf of the Attendee and on behalf of our respective personal representatives, executors, heirs, agents and assigns, release Stevenson University, as well as its directors, trustees, officers, employees, instructors and agents from all claims while on university property and/or resulting from the Attendee’s participation in the camp  including claims due to illness injury, death or other damages. This waiver of liability includes claims related to Attendee’s travel to or from the site of the camp/clinic. In the event of the Attendee’s illness or injury during the activity, I hereby authorize the staff members of the university to obtain medical assistance and consent to treatment for the Attendee. In the event that medical treatment is provided, I will not hold Stevenson University, its employees, officers or agents responsible or liable for the judgement of and/or treatment by the physician.  I understand that Stevenson University, and the Department of Athletics or their employees are not financially responsible for medical, dental, or other expenses incurred as a result of the Attendee’s participation in the camp/clinic.
